Privacy Policy & Terms of Use

1.     The purpose of nutritional counselling is to improve the overall health, vitality and well-being of the body through nutritional education and the use of natural foods and non-medicinal nutritional supplements. The Registered Dietitian, Anna Prestia, does not diagnose diseases, disorders or conditions. The Registered Dietitian, Anna Prestia, is not a Medical Physician.

 

2.      As part of the Nutritional Counselling Services, I may be asked to provide information concerning my physical habits, medical history, moods, energy levels, likes and dislikes, lifestyle and diet. This information is collected to enable the Anna Prestia RD to: (i) assess my knowledge of nutrition, (ii) education me about the benefits of sound nutritional practices and (iii) recommend dietary changes to improve my general health, vitality and overall

well-being. Anna Prestia RD will hold this information in confidence and will not release or disclose this information to any other person, without my prior consent, except as required by applicable law.

 

3.     If Anna Prestia RD suspects the existence of disease, disorder or condition, I will be informed of this suspicion. However, I acknowledge this is not a diagnosis or conclusion about the state of my health and that I am directed to promptly consult a medical Physician.

 

4.     Should I request that Anna Prestia RD recommend dietary changes and/or nutritional supplements to enhance my body’s natural ability to resist and/or overcome a known disease, disorder or condition, it is my responsibility to disclose the nature of the disease, disorder or condition and all other relevant details to Anna Prestia RD. If I have not previously consulted a medical Physcian about this disease, disorder or condition, I acknowledge that I am directed to promptly do so. I am not to alter or discontinue treatments prescribed by a medical Physician or other licensed health professional without consulting the individual who prescribed the treatment.

 

5.     In providing Nutrition Counselling Services to me, Anna Prestia RD is relying upon the truth, accuracy and completeness of all information I have provided to her. Any recommendations I follow for changes in diet, including the use of nutritional supplements, are entirely my responsibility.

 

6.     Anna Prestia RD is in no way liable for my health or safety.

 

7.     If you need to cancel an appointment, 24 hours notice is required and greatly appreciated.  There is a $25.00 fee should you miss your appointment or cancel without giving the required notice.  Extenuating circumstances will be considered.  To be fair, should I need to cancel without the 24 hours notice to you, you will have a deduction of $25.00 from your next session.  

 

8.     Registered Dietitians in Ontario are members of the College of Dietitians of Ontario (CDO) which regulates the practice of Registered Dietitians in Ontario.  The CDO ensures that all registered members are competent and that they abide by a code of ethics and standards of practice set out by the regulatory body.

 

9.     Innovative Nutrition Therapy has taken steps to make sure all information received from our clients and online visitors, including personal information, username, password, transaction information is as secure as possible against unauthorized access and use. We take your privacy seriously, and make every reasonable effort to protect it. Please note that data transmission over the Internet is never 100% secure. Therefore, we cannot guarantee the security of the information that you transmit via our online services. By using this website, you consent to the acceptance of this policy. This policy and all legal obligations of Innovative Nutrition Therapy are subject to the laws of Ontario and Canada.

 

10.    Confidentiality is an essential aspect of counselling and is required to build trust.  All services provided to you are strictly confidential.  Furthermore, any documentation pertaining to you and your treatment will be kept confidential in accordance with the College of Dietitians of Ontario.  Additionally, you are permitted to access your treatment file (with prior arrangement), however, you must provide written consent if any of your information is to be shared and released to a third-party. Information about you will not be released without your informed written consent except in the following circumstances: Innovative Nutrition Therapy may disclose your personal information: (a) as necessary to provide services to you the client with your knowledge and permission; (b) to any governmental authority as part of an investigation to determine our compliance with any applicable law, rule or regulation (including privacy laws, rules and regulations); (c) in response to a court order or subpoena  (d) as otherwise required by any applicable law, rule or regulation; and (e) to the extent required to permit us to investigate suspected fraud, harassment or other violations of law or our website rules or policies.

 

11.     I understand that any therapies I undertake at Innovative Nutrition Therapy are undertaken of my own free will. I accept that the ultimate responsibility for my health care is my own and that Innovative Nutrition Therapy is here to support me in this. I understand that my practitioner reserves the right to determine which cases fall outside their scope of practice, in which event an appropriate referral will be recommended.

12. Ownership of Website and Content:

Innovative Nutrition Therapy is the registered, exclusive owner of the intellectual properties, title, right and content  displayed on the website located at https://www.innovativenutritiontherapy.com. 

Visitors to this website are not granted ownership of any of the content accessed  on this website.  Any content and intellectual property may not be removed or altered.  Visitors/clients may not distribute, modify, reuse, repost, or copy  any of the content for commercial purposes.
